May 2021 Testing Refusal Flowchart

• Families can refer to the testing flowchart linked in the above heading to guide their decision about testing refusals.
• High School students who have already met the graduation and federal participation testing requirements necessary for graduation will not test and do not need to complete the refusal form.

Completing and Submitting the Testing Refusal Form

• You will only need to complete one form for each student, but list each SOL refused individually.
• STI numbers will be filled in by the school.

Step 1

Please complete the appropriate school level Google form for each student no later than April 26, 2021:

• Middle School Students: LBSS SOL COVID Refusal Intention Form for Middle School
• High School Students: LBSS SOL COVID Refusal Intention Form for High School

Step 2**

Please complete the COVID Refusal Form by May 21, 2021. Choose one of the following options to submit the refusal form:

• Return a copy to your student's math teacher
• Mail to the school at: LBSS Testing Office 9200 Burke Lake Rd Burke, VA 22015
• Send a scanned copy of your signed form to @email 
• @email  (a confirmation email receipt will be automatically sent)

** Please note:

• The school will complete the STI #.
• Parents need to sign the form, but LBSS staff will gather additional signatures and send to the appropriate county officials.
• DO NOT send forms to the county employee address listed at the bottom of the paper/PDF form.
• If your student is refusing a Federal Participation ONLY test, there will be no make-ups nor will it affect on-time graduation (Agreement #2). Refer to this flowchart and the video below about Federal Participation only SOL tests.
