Parents and Students,
Welcome to the 2018-19 school year! We hope that you have had a restful summer break and are getting ready for the first day of school (Tuesday, August 28, 2018).
The most frequently asked question about the Back to School forms is, "Which papers do I have to return to the school to get my locker?" The number of forms that must be returned depends on if you are a middle or high school student.
- All students/parents must complete and return the Emergency Care Card (sent in the US Mail), the health information form, and the FCPS Student Rights & Responsibilities form.
- Middle school students/parents must also complete and return the Middle School After School Program Registration form.
All of these forms can be found under the Required Documents link below and can be returned at the Bruin Blast on Thursday, August 23, 2018, between 9:00 a.m. – 11:00 a.m. (Middle School) and 1:00 - 3:00 p.m. (High School).
- Optional forms, including medication authorization and opt-out forms, can be found in the links below.
Registration and membership forms for many of our parent and student organizations are also included in the links below. These completed forms may be returned at Bruin Blast or sent to the school with your student (drop-off can be made in the main office or the subschool) on the first day of school.
